The BAARS-IV Other Report Childhood Symptoms form is an informant-based rating scale used to gather retrospective information about an adult's ADHD-related behaviors during childhood. It is completed by someone who knew the individual during their childhood and can provide information about earlier behavioral patterns. The form contributes collateral developmental information relevant to evaluating the history and possible childhood onset of ADHD symptoms.
